Once it is filled out you can save the PDF then attach it to the email. If you don't see the option to save select print and set your printer as "Print to PDF" as shown below (The look and wording may vary slightly based on your computer setup), hit print and it will ask you where you want to save the file. Once saved it can be attached in the email.
